Steps:

  1. Complete the form: Fill out the employee’s information section of the form. This will typically include the employee’s name, job title, dates of employment and unique identifier information.
  2. Indicate the type of verification: Provide the type of information you are seeking. If type of request you are seeking is not listed in the available choices, please use the other section:
  3. Attachments: All verifications of employment need to have an employee release authorization request form. If you need for us to complete a verification form or financial information form. Please attach it to your request. Incomplete requests will not be processed.
  4. Provide your contact information: Please ensure to include your name, your job title, company name, email address and phone number. This will be utilized to provide the responsive information.

Responses: All employment verification requests will be completed and distributed via email. You will receive confirmation email form Human Resources, upon successfully submitting the form request.
