Steps:
- Complete the form: Fill out the employee’s information section of the form. This will typically include the employee’s name, job title, dates of employment and unique identifier information.
- Indicate the type of verification: Provide the type of information you are seeking. If type of request you are seeking is not listed in the available choices, please use the other section:
- Attachments: All verifications of employment need to have an employee release authorization request form. If you need for us to complete a verification form or financial information form. Please attach it to your request. Incomplete requests will not be processed.
- Provide your contact information: Please ensure to include your name, your job title, company name, email address and phone number. This will be utilized to provide the responsive information.
Responses: All employment verification requests will be completed and distributed via email. You will receive confirmation email form Human Resources, upon successfully submitting the form request.
