Submission Requirements
To ensure your request is reviewed promptly, all submissions must include the following:
- Completed Forms
All forms must be filled out in their entirety.
- Job Assessment Tool (JAT)
A completed JAT must be attached for all new positions or changes.
- Updated Organizational Chart
Include an org chart that clearly reflects and highlights the requested changes.
- Department Head Signature
Forms must be signed by the Department Head before submission.
